We have amended the process to request support from our home visiting clinicians (GPs, Advanced Clinical Practitioner (ACP) & Paramedics)

How to make a request

You can complete an online request through PATCHS, or someone else can do this on your behalf. You can do this through the NHS App, or via the link on the practice website.

If you are unable to do this, you can call the Practice (0113 268 0066 / 0113 249 0598).

  • Press 1 to put in your request via Telephone Assistant
  • Press 2 to speak to our reception team who can complete the request for you

We have updated our processes so that calls from our housebound patients are directed to the front of the call queue, so please make sure that we have the correct telephone number recorded.

You will be asked the same questions no matter which way you access the practice. This means that we can manage all requests and ensure that we provide can everyone with the most appropriate care and support.

We understand that some patients are concerned about using PATCHS to submit requests. We would encourage patients to use PATCHS if they can. Feedback is that is easy once the account is set up, and it is gives us more time to spend with patients who do not have online access or struggle for other reasons. However, as outlined above, this is not the only way to contact the practice.

What happens next?

Your request will be passed to a clinician to review.

One of our home visiting clinicians may send you a message to give you advice or get more information; or telephone you to discuss your concerns; or arrange a home visit to see you.

If the request is received before 12.30pm you will be contacted on the day.

If the request is received after 12.30pm you will be contacted the next working day unless the clinician assesses your request as being urgent, in which case you will be contacted on the day.

Why 12.30pm for same day requests?

This is to allow us to safely assess your request and so that our home visiting clinician (GP, Advanced Clinical Practitioner (ACP) or Paramedic) can plan their visits and provide the best service to all our patients.
